Physical Exams
Requirements
New York State requires a physical exam for all new students and students in grades pre-K or K, 1,3,5,7,9 and 11. Physical exams are also required annually for interscholastic sports, as needed for working papers or as required by the Committee on Special Education (CSE) or the Committee on Pre-School Special Education (CPSE).
A School Health Examination Form must be completed and signed by a state licensed physician, physician assistant or nurse practitioner within 30 days of a student starting the grades listed above. The date of the exam must not to be more than 12 months prior to the beginning of the school year in which the examination is required. A notice will be sent to parents/guardians of any student who has not provided the required form.
